  • Click on the link below for instruction on making a hair replacement base template.
  • Your hair color or a hair color number.
  • A color sample is best to get a perfect match, but you can select a Hair DIRECT from our human hair color chart.

  • 2.Complete the form below and mail it with your template, and hair color samples to the address below.

  • 3. Once we receive your template and hair sample we will contact you.
